1x1 pixel spacer Home health agencies serve a vital role in the healthcare community, providing care that enables homebound patients to remain independent in their homes.

Approach

Quality Partners' Safe Transitions Project works in partnership with 20 home health agencies to:
  • Promote seamless care coordination from the hospital to home health care,
  • Implementing related best practices, and
  • Ultimately, reduce unnecessary hospitalizations and re-hospitalizations that can be harmful to patients. 
The Safe Transitions Project is part of our work to support Medicare's National Patient Safety Initiative (NPSI) and to ensure safe care for Rhode Island beneficiaries.

Experience

Quality Partners’ associates include professionals with experience in home health, maternal child health, pediatrics and geriatrics. Our home health team also has expertise in outcome-based quality improvement. This methodology evaluates patient outcomes and provides methods for home health agencies to use to improve care in a measurable manner.
