Quality Partners of Rhode Island (Quality Partners) assists nursing home leadership and staff improve residents’ quality of care and quality of life, as well as introduce individualized care and develop solutions for workforce challenges.
Locally, projects led by Quality Partners have resulted in improved clinical outcomes and a higher quality of life for nursing home residents. Nationally, we have designed tools, written materials and led projects to help nursing homes across the country improve the quality of their services.
Approach
As the Quality Improvement Organization for the state of Rhode Island under our contract with the Centers for Medicare & Medicaid Services, Quality Partners works in partnership with 62 Rhode Island nursing homes to:
- Reduce Rates of Pressure Ulcers
- Reduce Rates of Use of Physical Restraints
- Improve Drug Safety
- Promote Seamless Transitions from the Hospital to the Nursing Home and from a Skilled Nursing Facility to a Home Health Care Agency

In addition to our CMS work, Quality Partners works directly with individual nursing homes as well as nursing home corporations (under private contracts) to improve quality of care and quality of life for nursing home residents by:
