Through our collaboration with our partners, Quality Partners of Rhode Island (Quality Partners) developed the Holistic Approach to Transformational Change (HATCh) model as a guiding philosophy to assist with a nursing home's culture change journey. This organizational culture change model includes six inter-related topic areas (domains) that lead to individual, organizational, community and systems changes.
HATCh considers these six inter-related domains that lead to personal, organizational, community and systems changes, all of which are necessary for a transformation from institutional to individual care. The center domains are overlapping areas of Workplace Practice, Care Practice and Environment. Leadership surrounds them most immediately. Each nursing home is of course encircled by Family and Community and then by Regulatory/Government domains.
The decision to adopt each of these domains, along with the interconnectedness of the domains, was carefully considered and key to facilitating change.
General Resources & Information for Individualized Care
- Improvements in each area contribute to positive results for residents, staff and families. We created the Individualized Care Training Curriculum that outlines the training materials and educational resources to help guide nursing homes in the adoption of innovative, transformational-change strategies.
- The Rhode Island Department of Health and Quality Partners are collaborating on the Individualized Care Pilot to provide support to Rhode Island Nurisng Homes. See the tools used by these homes to make successful changes
- Low Cost Practical Strategies to Transform Nursing Home Environments: Towards Better Quality of Life
Changing the existing physical environment of nursing homes in ways that improve residents’ quality of life and without major expenditures can happen. It simply requires a commitment to improve the living environments for all residents and to become an actual home for the residents within the larger community of the overall facility.

- RI Generations is the Rhode Island culture change Coalition whose aim is to help Rhode Island nursing homes move from an institutionalized to an individualized care model by sharing information and ideas, working to educate local stakeholders (e.g. educational institutions, state surveyors) and affecting state policy and regulations.

This study, funded by the Centers for Medicare & Medicaid Services in 2004, integrated quality improvement practices with individualized care and workforce retention.
