Quality Partners of Rhode Island (Quality Partners) works with primary care and specialist physicians and their office staff to implement state-of-the-art tools and care delivery through:

We help support local implementation of physician-focused quality improvement, system and care process redesign, patient self-management and care coordination.
Approach
As the Quality Improvement Organization for the state of Rhode Island under our contract with the Centers for Medicare & Medicaid Services (CMS), Quality Partners works in partnership with a defined group of primary care physician offices to improve preventive health care services.

In addition to improving preventive health services, Quality Partners works with primary care physician offices on the clinical area of chronic kidney disease (CKD).

Experience
Our staff includes professionals with experience in electronic medical record implementation, case management and the chronic care model, adult learning principles, quality measurement and reporting, advance access scheduling and other care redesign tools and techniques. We works with physician practices to improve chronic care management by using the Chronic Care Model (CCM). The CCM is a population-based model focused on six components:
- The Health Care Organization
- Community Resources and Policies
- Self-management Support
- Decision Support
- Delivery System Design
- Clinical Information Systems
These main components of the CCM help physicians know which patients have a chronic disease, ensure patients receive evidence-based care, and enable patients to actively participate in their care.
