During the past three decades, the incidence and prevalence of Chronic Kidney Disease (CKD) have risen progressively. 

The National Kidney Foundation (NKF) has estimated that 20 million Americans have early CKD and do not know it. Further, another 20 million face an above average risk of developing it. 

Quality Partners of Rhode Island (Quality Partners) is one of ten QIOs nationwide that was awarded a three-year contract to improve the detection and treatment of CKD in patients with diabetes.

Quality Measures

The Quality Measures that guide our work include:  

  • Early Screening to Detect the Rate of Kidney Failure Due to Diabetes. Our goal is to increase the adoption of evidence based standards to identify CKD in Medicare patients through an annual urinary microalbumin measuremnt for all individuals with diabetes. 
  • Slowing the Progression of Kidney Disease in Hypertensive Individuals With Diabetes Through the Use of an ACE Inhibitor and/or an ARB Agent. Our Goal is to increase the frequency with which individuals with diabetes and earlier stage CKD (stage 1-4) are treated with and ACE inhibitor and/or ARB agent for the treatment of hypertension.
  • Timely Referral to Nephrology and Vascular Surgery to Ensure That a Mature Fistula is in Place at the Time of Initial Hemodialysis. Our goal is AV fistula placement and maturation as a first choice for arteriovenous access where medically appropriate for individuals who elect hemodialysis as their treatment option for kidney failure.

Strategic Coalition

The CKD Strategic Coalition provides a forum for stakeholders to work together to improve the quality of care and medical outcomes, including access to care, for patients at risk for and/or diagnosed with CKD. 

The Coalition consists of stakeholders who will identify priorities at the system level and develop recommendations that can be implemented in all settings.

Practice Assistance Program

Quality Partners developed a Practice Assistance Program to support and guide primary care practices in working with individuals who have or are at risk for developing chronic kidney disease. This program is grounded in the Patient Centered Medical Home (PCMH) model of care and incorporates health information technology as a tool to assist with outcome improvement. 

Practice specialists also offer support with E-Prescribing

Advisory Panel

To support this Practice Assistance Program, Quality Partners assembled a CKD/DM Advisory Panel. 

The Panel, comprised of Endocrinologists, Nephrologists, Primary Care Physicians, Clinical Pharmacists, Registered Dietitians and a Vascular Surgeon, will provide guidance on key areas of opportunity.

The Advisory Panel, along with support from the Strategic Coalition, has provided guidance on provider engagement and educational sessions.

Quality Partners of Rhode Island hosted a physician event: A Dinner Meeting with a Nephrologist Panel:  Screening for and Treating Chronic Kidney Disease in Patients with Diabetes - Addressing Controversial Issues with featured speaker, Alan Kliger, MD. 

Panelists included Andrew Brem, MD, Charles McCoy, MD, Douglas Shemin, MD and David Yoburn, MD.

  • American Association of Kidney PatientsFounded by kidney patients for kidney patients, this organization strives to provide education and resources that support the health and wellbeing for people with kidney disease.
  • American Diabetes AssociationPatients will find extensive information and resources on diabetes.
  • Kidney School: Aimed at kidney patients in stages 3-5 of kidney disease, this site offers free, up-to-date, research-based information packaged in 20-minute educational sessions.
  • The National Kidney Disease Education ProgramPatients will find extensive information and resources in English and Spanish on this site, including easy-to-read informational brochures and fact sheets.
  • The National Kidney Foundation: This nonprofit organization provides patient and community services for people affected by kidney disease as well as supports preventive initiatives.
