The Centers for Medicare & Medicaid Services' (CMS') new three-year NPSI focuses on helping providers improve patient safety through two related projects: Safe Care Project Since the release of the Institute of Medicine’s report, "To Err is Human" (1999), which estimated that up to 98,000 patients die each year due to care lapses, patient safety has become a pressing national challenge.
